Depression in Persons with Parkinson's Disease: Clinical Perspectives and Management

Depression affects over one-third of Parkinson's disease patients with significant diagnostic challenges due to symptom overlap between the conditions. Treatment involves pharmacotherapy (SSRIs, SNRIs, tricyclics) and psychotherapy (CBT, interpersonal therapy) adapted for PD-specific vulnerabilities, though access to specialized mental health care remains limited.

Multi-session neurofeedback with implanted deep brain stimulation electrodes in Parkinson's disease patients

This exploratory study investigated the feasibility of repeated neurofeedback sessions using implanted DBS systems in 8 Parkinson's disease patients, where real-time visual feedback of beta oscillations from the subthalamic nucleus allowed patients to attempt regulation of neural activity across three separate days. Results showed significant beta power reduction in sessions 1 and 3, with dopaminergic medication negatively impacting downregulation performance while DBS stimulation enhanced it.

Glucagon-like peptide-1 receptor agonists in movement disorders: Mechanisms, evidence, and future directions

GLP-1 receptor agonists, originally developed for diabetes and obesity, show neuroprotective properties in preclinical models of movement disorders by modulating neuroinflammation, oxidative stress, and protein aggregation. Clinical evidence is most advanced in Parkinson's disease, though early promise from exenatide and lixisenatide trials was not confirmed in larger phase III studies, with preliminary signals emerging in other movement disorders.

Brain Cholinergic Imaging in Isolated REM Sleep Behavior Disorder: Distinctions and Similarities with Parkinson's Disease

This PET neuroimaging study examined cholinergic terminal density in 16 patients with isolated REM sleep behavior disorder (iRBD, a prodromal stage of Parkinson's disease) compared to 12 Parkinson's patients and 18 controls. Both patient groups showed similar cortical cholinergic denervation (11-12% reduction) with distinct regional patterns, while iRBD showed associations between lower cholinergic uptake and impaired executive/visuospatial function.
