Cerebrovascular Events After Head and Neck Radiation Therapy: Systematic Review and Meta-Analysis

This systematic review and meta-analysis of 50 studies demonstrates that head and neck cancer patients receiving radiation therapy have a significantly increased risk of ischemic stroke (8.98 per 1,000 person-years) and carotid stenosis compared to surgical or non-treated populations. The relative risk of stroke is approximately 1.3 times higher in irradiated patients, with carotid stenosis incidence ranging from 0-40% depending on follow-up duration.

Beyond Anticoagulation: Understanding Medical Therapy Failure in PFO-Related Stroke and the Case for Closure

This narrative review examines why anticoagulation fails in some patients with patent foramen ovale (PFO)-related cryptogenic stroke, synthesizing evidence showing transcatheter closure is superior to medical therapy in appropriately selected patients. The analysis incorporates pathophysiological mechanisms, randomized trial comparisons, and risk stratification tools (RoPE score, PASCAL classification) to guide optimal therapeutic selection.

Pre-stroke sleep quality predicts 1-year mortality and functional outcomes in ischemic stroke and TIA patients

This cohort study of 2,266 stroke/TIA patients found that poor pre-stroke subjective sleep quality (highest PSQI quartile) significantly increased 1-year mortality risk (3.73-fold) and poor functional outcome risk (2.15-fold) compared to good sleep quality. The association remained independent after adjusting for confounders, suggesting sleep quality as a prognostic marker in acute ischemic stroke.

Cervicocerebral atherosclerosis and atrial fibrillation increase long-term mortality in ischemic stroke patients

This study of 406 acute ischemic stroke/TIA patients found that atrial fibrillation combined with high cervicocerebral atherosclerotic burden significantly increased 7-year mortality risk, with those having AF and high atherosclerosis burden facing approximately 3-fold greater mortality risk. The findings suggest that concurrent presence of these two vascular conditions substantially worsens long-term outcomes compared to either condition alone.

Expression of Concern: Ischemic Stroke and TIA Related to Vasculitis Secondary to Primary Sjögren's Syndrome

This is an expression of concern (editorial notice) regarding a previously published case series linking ischemic stroke and transient ischemic attack to large-vessel vasculitis complications of primary Sjögren's syndrome. The concern document does not appear to endorse or dispute the clinical relationship itself, but rather raises methodological or publication integrity questions about the original article.

Association Between TSH Level and Stroke Severity: A Cross-Sectional Study Using the Modified Rankin Scale

This cross-sectional study of 204 stroke patients examined the relationship between thyroid-stimulating hormone (TSH) levels and stroke severity measured by the modified Rankin scale. A statistically significant weak-to-moderate negative correlation was found between TSH levels and stroke disability during admission and discharge, suggesting thyroid dysfunction may influence stroke outcomes.

Post-discharge telephone follow-up for ischemic stroke survivors: implementing best practices at a comprehensive stroke center

This quality improvement project implemented standardized post-discharge telephone follow-up protocols for ischemic stroke and TIA patients using the JBI Evidence Implementation Framework, addressing barriers through a multidisciplinary approach with updated protocols and staff education. Results showed statistically significant improvements in compliance with evidence-based follow-up practices compared to baseline measurements.
