Baseline versus Longitudinal Cognitive Assessments for Diagnosing Alzheimer's Disease in Adults with Down Syndrome

This longitudinal study of 562 adults with Down syndrome compared the diagnostic accuracy of baseline cognitive testing versus one-year cognitive decline for detecting symptomatic Alzheimer's disease. Baseline assessments using CAMCOG-DS and mCRT showed superior diagnostic performance (AUC 0.82-0.99) compared to one-year intra-individual decline measures, though longer follow-up periods improved the utility of decline-based assessments.
