Post-discharge telephone follow-up for ischemic stroke survivors: implementing best practices at a comprehensive stroke center

This quality improvement project implemented standardized post-discharge telephone follow-up protocols for ischemic stroke and TIA patients using the JBI Evidence Implementation Framework, addressing barriers through a multidisciplinary approach with updated protocols and staff education. Results showed statistically significant improvements in compliance with evidence-based follow-up practices compared to baseline measurements.
