This study identified altered microRNA profiles in serum extracellular vesicles that distinguish ischemic stroke and transient ischemic attack patients from controls, with let-7b-5p and let-7f-5p emerging as key diagnostic markers. Mechanistic analysis suggests these miRNAs regulate FOXO1 and BDNF pathways involved in ischemic injury, supporting their potential clinical utility for stroke diagnosis and severity stratification.
