Intraindividual Cognitive Variability Predicts Amyloid-Beta, Tau PET, and Dementia Conversion in Down Syndrome

A study of 460 adults with Down syndrome found that greater intraindividual cognitive variability (fluctuations in cognitive performance) at baseline strongly predicted progression to mild cognitive impairment or dementia, amyloid accumulation, and tau pathology on PET imaging, independent of overall cognitive level. The findings suggest cognitive variability may serve as a sensitive biomarker for early Alzheimer's disease-related changes and guide secondary prevention strategies in this high-risk population.
